About the position

The Program Case Manager for Shelter/Programs at New Hope Housing Inc. is a full-time exempt position responsible for overseeing and coordinating case management services for residents in the Fairfax programs. This role involves supervising Shelter Case Managers, ensuring the smooth operation of case management services, and connecting residents with essential health and social services. The Program Case Manager will develop individualized housing stabilization plans, conduct assessments, and provide crisis counseling while adhering to the agency's core values of dignity, stability, and wellness.

Responsibilities

  • Supervise Shelter Case Managers and provide supportive supervision.
  • Coordinate and provide case management for guests/residents of the Fairfax programs.
  • Conduct psychosocial assessments to determine immediate needs and barriers.
  • Develop individualized housing stabilization plans addressing various life domains.
  • Refer, coordinate, and monitor the delivery of appropriate services to meet residents' needs.
  • Provide crisis counseling and make referrals for emotional and environmental problems.
  • Monitor guest/resident progress and adjust plans as necessary.
  • Accompany residents to appointments as needed.
  • Create and maintain confidential guest/resident records in compliance with agency policy.
  • Provide timely communication about program-related successes and concerns to the Director.

Requirements

  • Bachelor's degree in social work or related field or equivalent experience.
  • Extensive knowledge of social, economic, health, and behavioral problems.
  • Ability to identify social problems and assess residents' needs.
  • Strong knowledge of benefit programs and community resources.
  • Ability to establish effective working relationships and advocate for service populations.
  • Proficient in Microsoft Office and HMIS or similar database software.
  • Ability to communicate clearly and effectively both orally and in writing.

Nice-to-haves

  • Completion of college-level courses in sociology, psychology, or related human service fields.
  • Demonstration of commitment to the agency's mission and values.
