About the position

The Program Coordinator - Case Management plays a crucial role in supporting the RN Case Managers and Social Workers within the interdisciplinary healthcare team at the Medical University Hospital Authority (MUHA). Reporting directly to the Manager of Case Management, this position is designed to enhance the efficiency and effectiveness of case management operations. The Program Coordinator is responsible for coordinating payer communication, implementing transition plans, and ensuring the timely delivery of patient documentation. This role is essential in facilitating smooth operations within the case management department, thereby contributing to improved patient care and outcomes. In this position, the Program Coordinator will engage with various stakeholders, including healthcare professionals and patients, to ensure that all aspects of case management are executed seamlessly. The role requires a proactive approach to problem-solving and the ability to work independently while also collaborating effectively with team members. The Program Coordinator will be expected to manage multiple tasks simultaneously, demonstrating excellent organizational skills and attention to detail. The ideal candidate will possess a strong background in healthcare, particularly in patient billing, and will be proficient in medical terminology. The role demands strong communication skills, both verbal and written, as well as the ability to interact comfortably with patients and their families. Additionally, the Program Coordinator must be adaptable, as the position requires working alternating weekends and may involve various physical demands associated with the healthcare environment.

Responsibilities

  • Support RN Case Managers and Social Workers in their work with the interdisciplinary healthcare team.
  • Coordinate payer communication and transition plan implementation.
  • Deliver patient documentation and assist with other case management departmental functions.
  • Collaborate with medical professionals in a positive and professional manner.
  • Maintain accurate records and documentation related to case management activities.

Requirements

  • High school diploma or equivalent (GED) required; Bachelor's degree preferred.
  • Minimum of five years of relevant healthcare work experience, including experience in patient billing.
  • Proficiency in speaking, understanding, interpreting, and writing medical terminology.
  • Advanced knowledge of MS Office Suite, including Word, Excel, and PowerPoint.
  • Excellent communication skills and attention to detail.
  • Ability to work independently and collaboratively with medical professionals.

Nice-to-haves

  • Ability to speak Spanish is desired.
  • Experience in a healthcare setting is preferred.
