About the position

The Program Coordinator II (Social Worker) position at The Lundquist Institute focuses on coordinating and facilitating services within the Medical-Financial Partnership Program. This role is essential in ensuring continuity of care for families in prenatal, perinatal, and pediatric settings, while also promoting early relational health and addressing psychosocial needs. The coordinator will oversee team activities, provide direct clinical support, and collaborate with interdisciplinary teams to enhance service delivery and minimize gaps in care.

Responsibilities

  • Coordinate and supervise Medical-Financial Partnership team activities.
  • Ensure continuity of services and optimize service delivery.
  • Minimize gaps in care for families.
  • Oversee and support Medical-Financial Partnership services across care settings and departments.
  • Provide direct clinical support for the Medical-Financial Partnership interdisciplinary program.
  • Promote trust-based parent-provider relationships across care settings.
  • Assist in the collection of relevant clinical measures for patient care and program support.
  • Perform professional social work service to patients and families with complex medical and psychosocial needs.
  • Provide ongoing education and supervision of social work interns/resilience coaches.
  • Participate in the recruitment and placement of social work interns/resilience coaches.
  • Support facilitation of Medical-Financial Partnership multi-disciplinary team conferences.
  • Promote and maintain collaborative relationships with healthcare team members and community agencies.
  • Document clinical and program service activities accurately and timely.

Requirements

  • Master's Degree in Social Work from an accredited graduate school.
  • Two years of clinical social work experience.
  • Valid and active license as a Licensed Clinical Social Worker issued by the California Department of Consumer Affairs.
  • Registered Associate Clinical Social Worker registration must be obtained within 60 days of employment.

Nice-to-haves

  • At least 5 years post-degree experience working in a multidisciplinary team within a medical setting.
  • Experience evaluating infant, family, early childhood mental health and developing interventions/care plans.
  • Spanish language fluency.

Benefits

  • Salary up to $108,000 per year.
