About the position

The Program Coordinator for Persons with Disabilities Home is responsible for managing a full range of activities for individuals served in a group home setting. This includes ensuring the implementation of individual program plans and assigning staff duties as part of daily operations. The position is full-time and located in Muskegon, Michigan. The role requires experience in direct care within the mental health field and working with vulnerable adults.

Responsibilities

  • Assist in planning, conducting, and scheduling staff education classes, on-the-job training, and orientation programs.
  • Assist persons served with personal care and implement individual plans of service as needed.
  • Collaborate with assigned case manager/social worker and other professional support staff on the development and implementation of individual program plans.
  • Review and evaluate daily documentation of incident reports and program assignments.
  • Participate in establishing and meeting home's objectives relating to persons served.
  • Assign staff duties daily and monitor to ensure quality of service meets all division, agency, and regulatory standards.
  • Assume supervisory responsibilities in the absence of the home manager.

Requirements

  • Satisfactory completion of DCH Residential Program Training Requirements.
  • Attain and maintain all required certifications (i.e., CPR, First Aid, Blood Borne Pathogens, etc.).
  • Complete required Community Mental Health (CMH) Group home training.
  • Two years' relevant experience.
  • Ability to communicate effectively orally and in writing.
  • Effective leadership skills.
  • Ability to carry out written, oral, and diagrammatic form.
  • Ability to document accurately and completely.
  • Ability to read, write, and speak English with proper grammar, punctuation, and diction.
  • Ability to perform basic mathematical calculations.
  • Ability and willingness to work with vulnerable adults.

  • Valid unrestricted driver's license with a good driving record.
  • Proof of automobile insurance if required by program/center.
  • Flexibility to work weekends and evening hours.
  • On-call responsibility for assigned area.
