About the position

The Program Coordinator (Qualified Intellectual Disability Professional) at NCC Inc. is responsible for overseeing the administrative and clinical management of a group of apartments and group homes. This role involves supervising House Managers and ensuring that both waiver and non-waiver case management services are effectively delivered. The Program Coordinator applies best practices to ensure that clinical and environmental supports are in place, thereby providing quality services that are tailored to the needs of individuals with developmental disabilities. The position requires establishing and maintaining administrative practices for the apartments and group homes, as well as fostering positive relationships with referral and sponsoring agencies, along with regulatory bodies. In this role, the Program Coordinator will provide direct supervision, coaching, and feedback to House Managers and Residential Supervisors, ensuring they complete their job duties effectively. Responsibilities include preparing payroll for direct reports, monitoring staff performance, and conducting annual evaluations. The Program Coordinator will oversee the maintenance of apartments and homes to ensure compliance with regulatory requirements and support the lifestyle of the individuals served. Additionally, the coordinator assists in the Individual Service Plan (ISP) process, ensuring that all required assessments are completed and that the residential, medical, and clinical portions of the ISP are adequately addressed. The Program Coordinator will also conduct Person Centered Planning meetings, develop outcomes that enhance the quality of life for individuals, and ensure that staff are trained in all necessary topics. This includes First Aid, CPR, Individual Rights, and other mandated trainings. The coordinator serves as a liaison between families, guardians, and external agencies, maintaining all pertinent information related to individuals and coordinating service delivery and case management. The role also involves overseeing waiver authorizations, collaborating with day programs, and managing individual funds in accordance with NCC/DDA/DDS policies. The Program Coordinator is expected to attend all required meetings, trainings, and court appearances, and will be on call after normal business hours to address emergencies or concerns.

Responsibilities

  • Provide direct supervision, coaching, and feedback to House Managers/Residential Supervisors.
  • Prepare payroll for direct reports.
  • Monitor and assess staff performance and complete annual evaluations for House Managers/Residential Supervisors.
  • Ensure apartments/homes are maintained in proper condition to assure compliance with regulatory requirements.
  • Assist DDS/DDA in the Individual Service Plan (ISP) process and planning.
  • Oversee implementation of Individual Service Plan (ISP).
  • Complete all required assessments, including safety plans and evacuation plans.
  • Ensure Residential, medical, and other clinical portions of the ISP are completed for services provided or contracted by NCC.
  • Conduct Person Centered Planning meetings and develop outcomes that support Quality of Life.
  • Ensure staff are trained in all required topics, including First Aid, CPR, and Individual Rights.
  • Conduct relevant training as needed and oversee new staff orientation.
  • Serve as a liaison between families, guardians, and external agencies.
  • Maintain all pertinent information related to the individual.
  • Coordinate service delivery and case management for services provided or contracted by NCC.
  • Oversee all data collection and waiver authorizations.
  • Coordinate and collaborate with day programs and visit work locations at least monthly.
  • Secure and maintain transportation to and from various locations in the community.
  • Provide administrative supervision of nursing staff and oversight of medical services.
  • Assist with management of individual funds in accordance with NCC/DDA/DDS policies and procedures.
  • Complete quarterly reports and court reports.
  • Attend all required meetings, trainings, and court appearances.
  • Be on call after normal business hours to address any emergencies or issues of concern.
  • Prepare for internal/external monitoring, surveys, and audits.

Requirements

  • B.A. Degree in a Human Services Field with one year of specialized training and one year experience working directly with people with Developmental Disabilities.
  • At least 5 years experience in Human Services, with 3 years in supervisory and administrative roles.
  • Ability to effectively communicate orally and in writing with internal/external vendors/clients, staff, and management.
  • Ability to read and comprehend simple instructions, business correspondence, and memoranda.
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.
  • Ability to deal with problems involving several concrete variables in standardized situations.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Basic computer skills and knowledge of Microsoft Office.
