About the position

The Program Coordinator at Heading Home, Inc. is responsible for managing an emergency family shelter program aimed at assisting homeless families in achieving independent living. This role involves overseeing service delivery to families residing in the shelter, ensuring that their needs are met through effective case management and supportive counseling. The Program Coordinator will set the direction for Case Managers and work closely with House Managers to advocate for the residents' needs, ensuring appropriate service delivery and support for each family. In this position, the Program Coordinator will utilize Motivational Interviewing techniques to conduct in-depth assessments of client concerns, which may include financial, legal, mental health, substance abuse, employment, healthcare, and housekeeping issues. Collaborating with heads of households, the Coordinator will develop Individual Service Plans (ISP) that encompass educational, vocational, medical, life skills, and housing goals. Regular meetings with families will be held to review ISP progress and plan next steps, ensuring that all clients receive the necessary resources and services to overcome barriers to housing. The Program Coordinator will also be responsible for navigating housing application processes, developing partnerships with internal and external service providers, and assisting clients with ongoing housing search activities. This includes supporting clients during their move-in transitions and documenting all interactions using appropriate reporting mechanisms. The role requires direct supervision of house managers, availability for counseling and crisis intervention, and participation in weekly meetings with the Program Manager and local providers to address residents' needs. The Coordinator will also serve as a liaison with community-based organizations and complete necessary reports for the program.

Responsibilities

  • Manage an emergency family shelter program with a focus on service delivery to homeless families.
  • Conduct in-depth assessments using Motivational Interviewing to determine immediate client concerns.
  • Develop individual service plans (ISP) in collaboration with heads of households.
  • Meet with families weekly to review ISP progress and plan next steps.
  • Obtain resources and services to address barriers to housing, including CORI mitigation and credit repair.
  • Assist clients with navigating housing application processes and referrals to appropriate housing opportunities.
  • Develop partnerships with internal and external service providers to ensure comprehensive services for clients.
  • Assist clients with ongoing housing search activities, including contacting landlords and attending appointments.
  • Provide move-in transition support, including furniture assistance and budget support.
  • Document all client interactions using web-based data collection systems.
  • Coordinate the Shelter's programs and services, providing direct supervision to house managers.
  • Be available for counseling and crisis intervention, making referrals to community programs as needed.
  • Attend weekly meetings with the Program Manager and case conference team meetings with local providers.
  • Serve as a liaison with community-based organizations and attend necessary meetings.
  • Complete reports for the program and perform other tasks as requested by the Program Manager.
