The Program Coordinator will coordinate the support and services necessary to assist adults supported by Benedictine to achieve their goals, established according to preferences, abilities, and choices, in their Person-Centered Plans. This role is crucial in striving to achieve the mission of Benedictine, operating in a person-centered and ethical manner that always focuses on quality of life and the health, safety, and well-being of the individuals served. The Program Coordinator will manage a caseload of no more than 35 people, ensuring that each individual receives the necessary support tailored to their unique needs and aspirations. The position requires consistent attendance and punctuality, with the expectation to report to work on time and prepared to perform assigned duties throughout the scheduled work period. The Program Coordinator will meet with each individual and their family annually, and as requested, to assess their goals and desires for living, working, and leisure. This involves coordinating with other staff who are familiar with the individual prior to the Person-Centered Planning (PCP) meeting, preparing required assessments and reports, and presenting these reports during the PCP meetings to the team members involved in the individual's support. In addition to these responsibilities, the Program Coordinator will work closely with individuals, day and residential supervisors, and staff to achieve the goals established in the PCP. This includes providing individualized staff training as needed. Regular visits to homes and job sites are required to monitor progress, resolve issues, and maintain effective relationships, with a minimum of quarterly visits. The Coordinator will also submit quarterly updates on progress toward goals and review documentation maintained by direct support staff. Advocacy for the individuals is a key component of this role, ensuring they are working toward their chosen goals while balancing health and safety considerations. Collaboration with DDA Coordinators of Community Supports regarding services, supports, and funding is also essential, along with confirming the accuracy of service funding and approving plans within established time frames.
