About the position

The Program Manager for Behavioral Health at East Bay Community Action Program (EBCAP) is responsible for leading and directing the development of health services plans within the Integrated Health Home (IHH) Team. This role involves monitoring consumer status, providing feedback on staff performance, and supervising the health home team in wellness initiatives and health education groups. The position requires a Master's degree in Human Services and relevant clinical experience, along with managerial skills to oversee team operations.

Responsibilities

• Provide leadership and direction for the development of health services plans during treatment plan meetings.
• Monitor each consumer's status and response to health coordination and prevention activities.
• Provide feedback regarding staff performance.
• Direct staff regarding individual cases and supervise the health home team in developing wellness and prevention initiatives.
• Facilitate health education groups.

Requirements

• Master's Degree in Human Services with LICSW/LMHC certification.
• Three to five years of post-graduate clinical experience in rehabilitative mental health.
• 1-2 years of managerial and supervisory experience.
• Familiarity with RIACT2 regulations is a plus.
