About the position

The Program Manager for Community and Family Services is responsible for overseeing the Community and Family Services Program, which includes supervising employees who provide essential services to individuals in need. This role involves performing intakes for individuals seeking services, matching staff with individuals based on their needs, and ensuring that ongoing services are maintained for all clients. The Program Manager will work closely with other agencies and families within the community to facilitate referrals and ensure that individuals receive the support they require. In this position, the Program Manager will hire and supervise direct care staff, which includes recruiting, training, and matching staff with individuals. They will also be responsible for monitoring the progress of staff and providing feedback, evaluations, and terminations as necessary. The role requires performing intakes for individuals seeking services, which involves scheduling appointments, gathering pertinent information, and ensuring that all documentation is completed accurately. The Program Manager will also be tasked with matching trained staff with individuals based on their service needs and ensuring that follow-up is conducted to assess service satisfaction within 30 days of service initiation. Additionally, the Program Manager will assist with the completion of documentation for service billing, ensuring that individual records contain all required information and that event reporting is completed according to DMH guidelines. They will monitor authorizations for funding services and provide advocacy and follow-through when funding concerns arise. Monthly reports for all individuals served must be submitted to the appropriate agency by the deadline, and annual assessments for all individuals served are required. The Program Manager will also provide direct billable services, including transportation, to individuals and groups as needed. Coordination of care with other agencies is a critical aspect of this role, which includes maintaining communication and collaborative relationships with various stakeholders, including funding sources and individual families. The Program Manager will promote the growth of the Community and Family Services Program to funding entities, individuals, and community partners while operating within established budgets. They will direct the process of regular program evaluation to ensure alignment with program goals and objectives and report findings to the Director. Compliance with appropriate program standards, such as DMH and CARF, is essential, and the Program Manager must remain informed of these standards.

Responsibilities

  • Perform intakes for individuals seeking Community and Family Services Program services.
  • Match trained staff with individuals seeking service based on service need.
  • Assist with the completion of documentation for service billing and ensure individual records contain all required information.
  • Monitor authorizations for funding for services and provide advocacy when funding concerns occur.
  • Provide monthly reports for all individuals served to the appropriate agency by the deadline.
  • Provide annual assessments for all individuals served and attend support plan meetings as possible.
  • Provide direct billable services (including transportation) to individuals/groups as needed.
  • Coordinate care with other agencies and maintain communication with stakeholders.
  • Promote the growth of the Community and Family Services Program to funding entities and community partners.
  • Direct the process of regular program evaluation to program goals and objectives and report to the Director.
  • Ensure health and safety standards are followed at the Organization and community-based sites.

Requirements

  • Bachelor of Science or Bachelor of Arts in a related field.
  • 3 years of experience working with individuals with developmental disabilities (DD).
  • Supervisory experience preferred.
  • Class E Missouri Drivers License and proof of vehicle insurance.
  • Successful completion of a criminal history background check and driving record review.

Nice-to-haves

  • Knowledge of CARF standards and DMH regulations.
  • Experience in professional writing and documentation.
  • Ability to use computer programs like Zoom, Microsoft Word, Excel, and online documentation systems.
  • Qualification as a Degreed Professional Manager preferred.
