About the position

PHMC is proud to be a leader in public health and is seeking a Program Manager to oversee the operations of the Community to Home (C2H) program. This program aims to develop collaborative relationships with mental, behavioral, and physical health services, as well as social services organizations, to provide home-visiting services to families in the south-central rural counties of Pennsylvania. The target demographic includes children with special healthcare needs aged 0-21 and their families. The Program Manager will play a crucial role in interfacing with various stakeholders, including medical practices, pediatric clinic staff, managed care organizations, service providers, intermediate units, and families. This position is primarily remote but requires coverage across 16 counties in Pennsylvania, including both Northeast and South Central regions. The Program Manager will be responsible for the daily management of program activities, ensuring that contractual and grant-funded projects are implemented properly and that program deliverables are met in a timely manner. Establishing and maintaining strong relationships with funders, providers, and project staff is essential for effective communication and efficient program implementation. The manager will also initiate connections with pediatric providers and other potential referral sources, collaborating with other C2H vendors and the Department of Health Program Officer to assess and determine the most appropriate practices and resources across the state. In addition to program management, the Program Manager will supervise C2H Care Managers and Community Health Workers, conducting regular reflective supervision, case reviews, and oversight of client progress. The role includes supporting staff in defining priorities, deadlines, and opportunities for growth, as well as leading program hiring and overseeing training, particularly for community health workers. The Program Manager will also contribute to publicizing program outcomes and participate in various community events and meetings to represent the C2H program and PHMC.

Responsibilities

  • Provide daily management of the program activities.
  • Work with the Assistant Director and Family Services programs staff to ensure contractual-grant-funded projects are implemented properly and timely program deliverables are met.
  • Establish and maintain strong relationships with funders, providers, and project staff to ensure effective communication and efficient program implementation.
  • Initiate and maintain connections with pediatric providers and other potential referral sources throughout south-central and northeast PA.
  • Collaborate with other C2H vendors and the Department of Health Program Officer to assess and determine the most appropriate practices and resources across the State.
  • Work closely with C2H staff to build capacity that meets the needs of consumers and supports community health worker training.
  • Coordinate and/or conduct all initial intakes with newly referred families, including assessing both child and family needs, over the phone or in-person as needed.
  • Review and ensure accurate and timely program paperwork submission by staff before completing data entry.
  • Work with Assistant Director to assure goals and deadlines are met.
  • Assist with evaluations of the effectiveness of service.
  • Complete program reports and other written documentation.
  • Maintain accurate records according to grant funding and observe HIPAA requirements; generate data and reports appropriately.
  • Attend required meetings regularly by phone or in-person.
  • Keep abreast of up-to-date events and services available for children with special healthcare needs in Pennsylvania and share information and resources with staff as appropriate.
  • Review and integrate the latest ideas and concepts with Assistant Director to improve project delivery, content, and/or evaluation for the target audience.
  • Participate and initiate conversations that bring forth innovative ideas and concepts that align with Health Promotion Council's (HPC's) strategic plan in regularly scheduled team meetings.
  • Participate in grant writing tasks as needed by upper management to develop concepts and write proposals to grow organizational initiatives; contribute content expertise for grant proposals as needed.
  • Ensure educational materials are culturally appropriate for diverse populations, especially for individuals with limited reading and English skills.
  • Participate in HPC supervisor team meetings regularly.
  • Represent Community To Home (C2H) and HPC at various meetings.
  • Establish and maintain linkages with medical providers, community health, social service, education and legal service agencies, and other support services through effective communication between C2H staff, community members, and C2H partners.

Requirements

  • Master's degree in social work or closely relevant fields.
  • At least four years of demonstrated ability to effectively supervise and lead staff.
  • Demonstrated experience working with health care providers, and knowledge of issues related to maternal and child health, child development, child abuse prevention, and children with special health care needs.
  • Experience working with diverse populations and low-income individuals.
  • Must clear child abuse, criminal history check, and FBI clearance.
  • Ability to travel throughout Northeast and South-Central Pennsylvania.
  • Ability to acquire information about new technological systems, organizations, and practices.
  • Experienced in working with 'motivational interviewing'.
  • Experience in data collection/entry, evaluation monitoring, and needs assessments.

Nice-to-haves
