About the position

The Program Manager/Counselor at LifeLong Medical Care will oversee the SAMHSA MAT program at the TRUST Health Center in Oakland. This full-time, exempt position involves managing the integration of mental health and substance use services, providing direct counseling, and ensuring compliance with grant requirements. The role emphasizes collaboration with various stakeholders to enhance patient care and support a diverse community.

Responsibilities

  • Oversee integration of mental health, substance use, and MAT services for the SAMHSA MAT program.
  • Administer supervision of SAMHSA MAT program staff, including hiring, training, and performance evaluations.
  • Provide direct SUD services, including co-occurring counseling services to clients receiving MAT, with outreach at homeless encampments.
  • Support retention of patients with co-occurring and SUD disorders in care from Street Medicine and the Trust Clinic.
  • Collaborate with Trust site leadership to improve coordination of SUD care and services.
  • Perform needs assessments and coordinate with community partners for patient referrals.
  • Ensure compliance with grant reporting requirements in collaboration with the Evaluator/Research Manager.
  • Coordinate care with agency-wide MAT and Recovery Support programs to ensure quality and standardization.
  • Communicate tasks, timelines, and updates to SAMHSA MAT program staff.
  • Utilize data to inform decisions impacting the SAMHSA MAT program's future direction.
  • Ensure grant deliverables are met and provide feedback to stakeholders.
  • Assess program performance and complete PDSA cycles for continuous quality improvement.
  • Participate in required supervision, team meetings, and training sessions.
  • Perform other duties as assigned by supervisor.

Requirements

  • Registered or certified as an SUD Counselor or ASW.
  • Knowledge and experience in providing substance use and mental health treatment services to diverse, low-income populations.
  • Managerial experience preferred.
  • Demonstrated ability to work with a diverse staff and client population, including individuals with mental illness and substance use issues.
  • Excellent judgment and decision-making skills.
  • Highly proficient in MS Office (Word, Outlook).
  • Ability to work effectively under pressure in a positive manner.
  • Excellent interpersonal, verbal, and written communication skills.
  • Strong organizational, administrative, and problem-solving skills.
  • Reliable access to transportation.
  • Ability to abide by all conditions of employment set forth by federal and state laws regulating healthcare settings.

Nice-to-haves

  • Experience with Epic Electronic Health Record preferred.
  • Bilingual Spanish/English preferred.
