About the position

The Program Manager for the Enhanced Family Care Program at Community Bridges is responsible for overseeing the support of individuals with disabilities living in community-based settings. This role involves managing home care providers, ensuring compliance with regulations, and delivering quality services to enhance the lives of individuals served. The Program Manager will also play a key role in fostering community engagement and collaboration among team members, families, and external resources.

Responsibilities

  • Support individuals to achieve their dreams and goals within a community-based setting.
  • Oversee individual's home care provider(s) to ensure they are working as part of the service team.
  • Provide support to home providers to help understand individual's Individual Support Agreement (ISA).
  • Communicate with individual's service team collaboratively.
  • Review and complete monthly progress notes and medical documentation as needed.
  • Maintain and update medical, day program, and certification books in compliance with regulations.
  • Interview potential home providers to assess their fit for the program.
  • Attend trainings to maintain knowledge of best practices.
  • Encourage individuals to access community supports and resources.
  • Act as a positive role model to educate the community about individuals with disabilities.
  • Supervise and monitor support situations, including regular home visits.
  • Ensure documentation quality and compliance with agency policies.
  • Present the agency in a professional manner at all times.
  • Attend meetings and conferences as requested by supervisor.
  • Ensure compliance with state regulations and agency policies.
  • Provide responsive and clinically sound services according to individual needs.

Requirements

  • Bachelor's Degree preferred or Associate's Degree plus 2-3 years of experience.
  • 1-2 years of experience working with people who have a disability, with at least one year of supervisory experience.
  • Reliable transportation and a valid driver's license.
  • CPR Certification and First Aid training.
  • Experience in medication administration and safety plans.

Nice-to-haves

  • Experience in community engagement and advocacy for individuals with disabilities.
  • Knowledge of state regulations regarding home care services.
