About the position

The Program Manager III position at the Executive Office of Health and Human Services in Cranston, RI, focuses on assisting individuals with disabilities and older adults in transitioning from institutional settings to community-based living arrangements. The role involves overseeing housing transition components of the Money Follows the Person (MFP) program, ensuring successful transitions, and promoting independent living for participants.

Responsibilities

  • Develop and maintain relationships with community stakeholders, advocacy groups, and service providers to promote the rights and inclusion of individuals with disabilities and older adults.
  • Develop and implement strategies to identify and secure affordable, accessible, and integrated housing options for MFP participants.
  • Collaborate with local housing authorities, nonprofit organizations, and private landlords to establish partnerships and increase the availability of suitable housing units.
  • Provide support for the Rhode Island Home Stabilization Medicaid benefit.
  • Lead quality assurance and case clinical discussions for the Nursing Home Transition Program.
  • Represent the MFP program at meetings, conferences, and events related to housing transitions and independent living.
  • Assist in tracking and analyzing data for submission to the State of Rhode Island and The Centers for Medicare and Medicaid Services.
  • Outreach and educate community stakeholders through formal and informal group presentations.
  • Provide support to the Rhode Island 811 Program voucher program.

Requirements

  • Bachelor's degree in social work, public administration, housing studies, or a related field; Master's degree preferred.
  • Minimum of 3 years of experience in housing programs, disability services, or related fields.
  • Experience in tracking and maintaining data through Excel spreadsheets.
  • Strong project management, problem-solving, and decision-making abilities.
  • Ability to analyze data, write data and narrative reports, and maintain data records.
  • Comprehensive technology skills including use of Microsoft Office programs and case management software.

Nice-to-haves

  • Experience in program development, implementation, and evaluation.
  • Knowledge of Medicaid regulations and policies related to long-term services and supports.
  • Familiarity with affordable housing programs.
  • Bilingual or multilingual skills.
  • Experience in federal grant management.
