About the position

The ILP Program Manager at Holy Cross Services is responsible for overseeing the Independent Living Program, ensuring that services meet contractual requirements while directly supervising staff. This role involves supporting participants in advocating for their needs, connecting them to community resources, and attending various meetings and appointments to facilitate permanency planning.

Responsibilities

  • Provide supervision with staff based on contract requirements to assess and individualize work responsibilities, goals, and job tasks.
  • Organize and hold team/support meetings and individual supervision meetings with all staff for in-service training, case consultations, support, and feedback.
  • Provide training/education to MDHHS local offices about contract services provided.
  • Work in conjunction with the Director in developing and maintaining the integrity of programs in accordance with licensing rules and regulatory bodies.
  • Identify, recruit, and train community representatives to become collaborative members for contract-based services.
  • Serve as the point person for referrals and intakes, maintaining a referral log.
  • Develop and oversee day-to-day activities and program processes, completing maintenance and IT requests as necessary.
  • Ensure census is maintained as provided by management.
  • Assist with the development and continuous monitoring of tracking systems that measure performance outcomes.
  • Attend meetings and co-facilitate groups including non-standard working hours such as evenings and weekends in accordance with client's needs.
  • Schedule and monitor HRIS activities and performance reviews of all staff.
  • Ensure all staff are following agency policies, site procedures, and treatment plans of participants.
  • Implement Mi-Team practices in daily activities including teaming, engagement, assessment, and mentoring.
  • Provide 24-hour crisis on-call availability to treatment team as needed.
  • Transport participants on an as needed basis.
  • Perform other duties related to the essential duties of the position as assigned by one's supervisor.

Requirements

  • 2 Year Degree (Associate Degree)
  • Experience in program management or a related field
  • Strong leadership and supervisory skills
  • Ability to work flexible hours including evenings and weekends
  • Excellent communication and interpersonal skills
  • Knowledge of community resources and services

Nice-to-haves

  • Experience working with MDHHS or similar organizations
  • Familiarity with crisis intervention techniques
  • Previous experience in a supervisory role
