The Behavioral Health Professional plays a crucial role in providing quality assessments of health home clients' care needs and facilitating timely access to care. This position is responsible for overseeing the ongoing evaluation of behavioral health needs for clients enrolled in a health home panel. The professional will spend approximately 80% of their time on Behavioral Health Professional duties, 10% on Care Coordination duties, and 10% on Client Service Advising duties. The role requires a commitment to fostering a culture of timely care, superior customer service, and genuine compassion to meet the behavioral health needs of each individual. In this position, the Behavioral Health Professional conducts initial comprehensive assessments of both new and returning clients using agency-identified assessment tools and baseline measures such as PHQ9, SBRT, DLA-20, LOCUS, and others. They are responsible for completing initial care plans and providing oversight for care plan updates. The professional maintains clinical oversight of clients assigned to the Behavioral Health Home (BHH) and signs off on documentation from other team members as necessary to ensure quality of care and medical necessity. The role also involves triaging crisis situations, identifying high-risk clients, and assessing needs for special reports or applications. The Behavioral Health Professional collaborates with Care Coordinators to monitor client participation and progress in specialty care, reassessing needs at appropriate intervals and making recommendations as necessary. They are expected to maintain thorough and accurate documentation of services provided, assessments, evaluations, and progress toward treatment goals, adhering to clinical and administrative policies and procedures. Overall, this position is integral to promoting the mission, vision, and values of Mental Health Partners through effective interaction and communication with clients and staff.
