About the position

The Program Manager for the Pathways to a Healthier Bernalillo County initiative will oversee the administration and execution of a multi-stakeholder social determinants of health (SDOH) intervention program. This role involves managing program services, ensuring compliance with service contracts, and reporting to the program advisory group. The Program Manager will act as the primary contact for community partners and organizations involved in the program, which aims to improve health outcomes for high-risk and vulnerable populations through case management and resource linkage.

Responsibilities

  • Serve as the primary point-of-contact for the Pathways Program.
  • Oversee all program services and deliverables.
  • Ensure execution of service contracts and compliance with program requirements.
  • Report to the program advisory group and partner organizations.
  • Manage program administration and documentation.

Requirements

  • High school diploma or GED.
  • At least 6 years of experience, with 2 years in a management role directly related to the duties specified.
  • Completed degree(s) from an accredited institution may substitute for experience on a year-for-year basis.

Nice-to-haves

  • Experience with contracts and budget management.
  • Experience overseeing a large, multi-faceted program.
  • Experience working with Community Health Workers, Peer Navigators, or Peer Support Workers.
  • Experience with social or community health services.
  • Bilingual preferred.
  • Bachelor's degree in business administration, community health, public administration, or related field.
  • Experience with SmartSheets, purchasing, invoicing, reconciliation, and Pcards.
  • Experience in reflective supervision.
