About the position

The Program Manager for Transitions at Meridian Healthcare is responsible for overseeing the operations of group homes in assigned counties, ensuring that programs meet performance indicators and regulatory requirements. This role involves supervising Recovery Specialists and clinical staff, implementing evidence-based practices, and advocating for clients while coordinating care with community agencies. Responsibilities

• Direct and supervise transitions group home operations in assigned counties.
• Supervise the activities of Recovery Specialists and assigned clinical and support staff.
• Ensure transition programs meet and exceed contract performance indicators and legal regulatory requirements.
• Implement and monitor fidelity to evidence-based and best practices in clinics.
• Oversee management of facility and program, including client care, staffing, and program activities.
• Work cooperatively with internal and external referral sources to facilitate access to services.
• Serve as program and client advocate, consultant, and liaison with community agencies.

Requirements

• Bachelor's degree from an accredited college or university in psychology, counseling, social work, rehabilitation, or a human service field.
• Current Florida Driver's License with a clean driving record.
