About the position

The Program Service Coordinator RN in Pulmonology at Monument Health is responsible for coordinating patient flow and managing data related to quality and productivity within the department. This role involves collaboration with various healthcare professionals to enhance patient care, education, and program efficiency, while also ensuring compliance with quality and safety standards.

Responsibilities

  • Collaborates in the development and implementation of efficient systems to support patient safety, improve patient satisfaction, and enhance provider/program productivity.
  • Acts as a liaison between physicians, hospital clinics, nursing personnel, residents, patients, etc. to optimize patient education, care, and scheduling.
  • Assists with patient education and interventions for identified patients in accordance with policy, procedure, scope, and licensure.
  • Demonstrates effective fiscal management through cost containment measures while ensuring high quality, cost-effective care is being provided to patients.
  • Collaborates with medical staff, hospital/clinic personnel, and other members of the interdisciplinary team to promote quality outcomes, patient safety, challenge, innovation, and/or growth of services.
  • Participates in departmental and organizational committees/teams to assist in achieving identified goals as required.
  • Actively supports, implements, maintains, educates, and promotes organizational quality initiatives and goals.
  • Maintains ongoing knowledge of associated legislative, regulatory, and/or compliance issues impacting practice and ensures up-to-date practices.
  • Establishes and maintains applicable program/service related competencies, policies, procedures, etc. as identified.
  • Serves as the program/department's point person to obtain necessary information to actively facilitate and coordinate appropriate program/patient flow through the continuum of care and processes, both intra and interdepartmentally.
  • Coordinates and participates in processes targeted at the promotion of community health and patient education.

Requirements

  • Completion of a nursing education program that is approved by a board of nursing.
  • Basic Life Support (BLS) Certification - American Heart Association (AHA) within 60 days of hire.
  • Registered Nurse (RN) - South Dakota Board of Nursing.

Nice-to-haves

  • 5+ years of Registered Nurse Experience.
  • Bachelor's degree in Nursing.
