About the position

The Program Specialist position at Family Practice & Counseling Network is responsible for supporting the health, safety, and growth of individuals in the program. This includes managing individual support plans (ISPs), ensuring compliance with medical requirements, addressing programmatic concerns, overseeing individual finances, and incident reporting while adhering to funder regulations. The role emphasizes the quality of programmatic, health, and facilitative services to meet the needs of individuals served.

Responsibilities

  • Ensure compliance with HIPAA regulations and timely completion of required documentation.
  • Complete and submit incident reports accurately and timely, taking corrective action when necessary.
  • Participate in audits and ensure all license requirements of program participants are met.
  • Maintain client files, including annual assessments and Individual Support Plans (ISPs).
  • Develop and maintain a catalog of community resources and coordinate communication for consumers with various stakeholders.
  • Check PROMISe eligibility and notify the Director of any issues affecting funding and billing.
  • Coordinate consumer financial matters such as SSI, food stamps, and other financial accounts.
  • Train direct care staff on individual support plans and mentor them in daily implementation and documentation.
  • Create an empowering environment for individuals by developing skills and coordinating resources.
  • Schedule all consumers' medical, dental, psychiatric, and psychological services as per regulations.
  • Create ISPs and advocate for consumers, ensuring accurate documentation of service needs and goals.
  • Write instructional plans for implementing consumer goals and train staff in ISP implementation.

Requirements

  • Master's degree or above from an accredited college or university (Special Education desired) and 1 year of experience working with individuals with intellectual disabilities or autism.
  • Bachelor's degree from an accredited college or university and 2 years of experience working with individuals with intellectual disabilities or autism.
  • Associate's degree or 60 credit hours from an accredited college or university and 4 years of experience working with individuals with intellectual disabilities or autism.
  • Proficiency in Microsoft Word, Excel, and PowerPoint for reporting requirements.
  • Willingness to maintain a minimum of 24 hours of in-service training per calendar year.
