As a Project Coordinator at Horizon Health Services, you will play a crucial role in implementing the SBIRT (Screening, Brief Intervention, and Referral to Treatment) public health model within school settings and other program environments. Your primary responsibility will be to act as a liaison between the schools and Horizon Health Services, ensuring that SBIRT services are effectively delivered to adolescent students. You will become familiar with various screening tools used in this program, including CRAFFT 2.1+N, PHQ-A, and PHQ-9, and will coordinate the scheduling of school-based trainings and community meetings throughout the grant period. In this position, you will be responsible for communicating program information to schools and other organizations through newsletters and other communication channels. You will ensure that consent is obtained for referrals to school-based clinics and that all program-related documentation is accurately entered into databases and systems in a timely manner. Monitoring and alerting clinicians of upcoming reassessment dates will also be part of your duties, along with documenting appointment frequencies, treatment plans, and outcomes in the patient's electronic health record (EHR). You will perform outreach to families and providers as directed by the Project Director, ensuring that all patient records are complete and that follow-through is maintained for all SAFER participants. Collaborating with the Communication and Outreach Specialist, you will engage with underserved and minority groups to educate and involve youth in the SBIRT program. Additionally, you will assist in developing program-related materials and participate in community meetings relevant to the program. Your role will also involve troubleshooting any issues that arise during the program's implementation, all while maintaining confidentiality of sensitive information.
