About the position

As a Project Coordinator at Horizon Health Services, you will play a crucial role in implementing the SBIRT (Screening, Brief Intervention, and Referral to Treatment) public health model within school settings and other program environments. Your primary responsibility will be to act as a liaison between the schools and Horizon Health Services, ensuring that SBIRT services are effectively delivered to adolescent students. You will become familiar with various screening tools used in this program, including CRAFFT 2.1+N, PHQ-A, and PHQ-9, and will coordinate the scheduling of school-based trainings and community meetings throughout the grant period. In this position, you will be responsible for communicating program information to schools and other organizations through newsletters and other communication channels. You will ensure that consent is obtained for referrals to school-based clinics and that all program-related documentation is accurately entered into databases and systems in a timely manner. Monitoring and alerting clinicians of upcoming reassessment dates will also be part of your duties, along with documenting appointment frequencies, treatment plans, and outcomes in the patient's electronic health record (EHR). You will perform outreach to families and providers as directed by the Project Director, ensuring that all patient records are complete and that follow-through is maintained for all SAFER participants. Collaborating with the Communication and Outreach Specialist, you will engage with underserved and minority groups to educate and involve youth in the SBIRT program. Additionally, you will assist in developing program-related materials and participate in community meetings relevant to the program. Your role will also involve troubleshooting any issues that arise during the program's implementation, all while maintaining confidentiality of sensitive information.

Responsibilities

  • Act as a liaison between schools and HHS to implement SBIRT services with adolescent students.
  • Schedule and coordinate school-based trainings and community meetings throughout the grant period.
  • Communicate program information through newsletters and other available communications.
  • Ensure consent is obtained for referrals to school-based clinics.
  • Enter all screening, intervention, consent, and program-related documentation into databases in a timely manner.
  • Monitor and alert clinicians of upcoming reassessment dates.
  • Document appointment frequency, treatment plans, and outcomes in the patient's EHR.
  • Perform outreach to families and providers as directed by the Project Director.
  • Obtain screening tools in other languages and schedule interpretation services when needed.
  • Schedule referral appointments within reasonable lengths of time.
  • Ensure follow-through for all SAFER participants, especially for treatment referrals.
  • Maintain regular contact with external providers for treatment maintenance and follow-up screening information.
  • Collaborate with the Communication and Outreach Specialist to perform outreach to underserved sections and minority groups.
  • Help develop program-related materials for distribution to the focus population.
  • Attend community-based meetings relevant to the program with the Communication and Outreach Specialist.
  • Assist in putting partner agreements into place.
  • Participate in developing and submitting a behavioral health disparities impact statement.
  • Help develop a sustainability plan for integrating SBIRT into daily care and treatment protocols after the grant period.
  • Troubleshoot problems as they arise during the program implementation.

Requirements

  • Bachelor's degree or higher in Health Services Administration or a related field required.
  • Exposure to managing large-scale programs, ensuring deliverables are met, and adhering to strict deadlines while working with a team.
