About the position

Clifford Beers is a mental health clinic dedicated to serving children and families, with a mission to provide integrated services that address mental, physical, and social determinants to improve health, resiliency, and quality of life. The Project Coordinator position is part of the Project Longevity initiative, which aims to reduce gun violence in Connecticut's major cities. This role involves direct care for individuals and families referred by the Project Longevity Program, focusing on providing wrap-around and trauma-informed service coordination. The Project Coordinator will work closely with the Service Coordination team to ensure that participants receive tailored services that meet their unique needs. The responsibilities of the Project Coordinator encompass all aspects of case management, including initial assessments, identifying participant needs, case planning, and facilitating connections with the participant's support network and external providers. The role requires comprehensive knowledge of social services, community dynamics, and the political landscape in Greater New Haven. The Project Coordinator will also provide specialized support to individuals and families with complex needs, utilizing a family-centered approach that acknowledges historical trauma and employs trauma-informed and anti-racist practices. In addition to case management, the Project Coordinator will engage in community outreach, participate in Project Longevity events, and maintain meticulous documentation in Electronic Health Records. The position demands adaptability and responsiveness to the evolving needs of the program, as well as a commitment to continuous learning and professional development in the Wraparound Model and Project Longevity initiatives. The Project Coordinator will also champion a trauma-informed, anti-racist, and equity-focused approach to improve health outcomes for all participants.

Responsibilities

  • Drive program success by ensuring timely and accurate completion of standardized forms and adherence to established procedures.
  • Serve as a strategic liaison between Project Longevity teams, law enforcement partners, and community agencies to facilitate collaborative problem-solving and consultation.
  • Deliver high-quality, trauma-informed support services to assigned families using the Wraparound approach, both in-home and in the community.
  • Coordinate seamless case conferences with referring contacts and lead balanced Family Team Meetings to ensure comprehensive support.
  • Pursue Wraparound Certification through rigorous training modules to maintain model fidelity and expertise.
  • Actively participate in Project Longevity Call-ins, community meetings, and events to promote the program and build strategic relationships.
  • Provide direct care to adults, children, and families with empathy and cultural sensitivity.
  • Engage in continuous learning and professional development to enhance knowledge and skills in the Wraparound Model and Project Longevity initiatives.
  • Leverage technology platforms to streamline needs assessments, care planning, referrals, and documentation, ensuring data accuracy and compliance.
  • Champion a trauma-informed, anti-racist, equity approach to improve health and well-being outcomes for all participants.
  • Maintain meticulous and timely documentation in Electronic Health Records, ensuring compliance with regulatory standards.
  • Collaborate with the Project Longevity team to identify and address service gaps, ensuring program excellence and integrity.
  • Embrace additional duties with enthusiasm and dedication, contributing to the program's success and growth.

Requirements

  • Degree in a behavioral health or criminal justice field preferred (three years related experience).
  • Experience working in an in-home setting, criminal justice, and behavioral health.
  • Knowledge of social services, neighborhoods, political landscape, and community stakeholders throughout Greater New Haven.
  • Familiarity with the New Haven community and New Haven area resources.
  • Strong collaboration, organization, and community engagement skills.
  • Proven ability in care coordination, case management, and working with children and families.
  • Proven ability in complex problem-solving and critical thinking skills.
  • Comfortable with technology, provider referral, and member data systems; experience with electronic health/medical records a plus.
  • Team Management and/or Group Facilitation experience.
  • Documentation and Reporting experience; Tableau experience a plus.
  • Adaptability with cross-cultural and bilingual/multilingual community; bilingual preferred.
