About the position

The position involves providing information, training, and support to patients, clients, and families through community outreach, client referrals, and educational efforts. The role requires obtaining necessary information from applicants and their families, increasing program visibility through community initiatives, and assisting individuals and families in achieving wellness. The candidate will educate clients on physiological processes and available resources, ensure compliance with legal requirements, and provide backup support to the Director of Nursing. Confidentiality and teamwork are essential aspects of this role.

Responsibilities

  • Provides information, training, and support to patients, clients, and families.
  • Obtains required information by interviewing applicants and gathering documents.
  • Increases program visibility through community outreach and education initiatives.
  • Assists individuals and families in achieving and maintaining wellness.
  • Educates applicants/clients on physiological processes and resources.
  • Obtains assistance for applicants/clients by referring them to community programs.
  • Complies with federal, state, and local legal requirements.
  • Provides backup support to the Director of Nursing.
  • Maintains client confidence and protects operations.
  • Contributes to team effort by accomplishing related results.

Requirements

  • Must have a high school diploma or equivalent.
  • Experience in Public Health or Social Work is recommended.
  • Must have a valid driver's license, auto insurance, and a dependable vehicle.
  • Ability to communicate effectively in English and Spanish.
  • Oral and written communication skills.
  • Ability to exercise poise and tact in all contacts.
  • Supervisory skills and knowledge of diabetes management techniques.
  • Ability to interact with people in a culturally sensitive manner.
  • Ability to speak effectively in public forums and facilitate discussions.
  • High performance standards and fulfillment of continuing education.
  • Current certification in CPR within a reasonable timeframe after employment.
  • Ability to function in remote areas and sensitivity to cross-cultural medical situations.
