About the position

At P3 Health Partners, we are a physician-led organization dedicated to transforming healthcare from sickness care into wellness guidance. We are seeking a Provider Operations Coordinator (POC) who will manage operational support initiatives and oversee special projects focused on HEDIS quality gap closures, Risk Adjustment activities, and provider engagement. This role requires daily problem-solving, meticulous tracking, and coordination across departments to meet varying and changing requirements. The POC will provide operational support to clinics, engage with internal and external stakeholders, and may directly interact with patients for quality gap closure tasks.

Responsibilities

  • Build and maintain effective relationships with assigned Affiliate groups to achieve goals and key initiatives.
  • Serve as a liaison between the Affiliate groups and P3 local, regional, and national departments.
  • Advise and support affiliate practices with quality gaps and conditional documentation through outreach and data audits.
  • Assist affiliate practices with patient outreach and scheduling for visits.
  • Manage high/rising risk patients by ensuring coordination of care across all healthcare services.
  • Prepare for and participate in CSR/IDT/JOC meetings as requested.
  • Conduct data entry and maintain documentation for quality initiatives.
  • Educate patients and affiliates on preventative screenings and their importance.
  • Conduct in-home HEDIS gap closure for select measures based on experience and education.
  • Perform other duties as assigned.

Requirements

  • High school diploma/GED required; associate degree in related field or equivalent experience preferred.
  • 2+ years experience in healthcare-related fields required; clinical healthcare experience preferred.
  • Experience with Microsoft Word, Excel, PowerPoint, Outlook, and general office equipment required.
  • Experience in Electronic Health Records required.
  • Medical Assistant Certification or experience as a Pharmacy Tech, Radiology Tech, Lab Tech or similar strongly preferred.
  • Experience with data mining preferred.
  • Knowledge of medical terminology required; experience with CPT II codes preferred.
  • Current CPR Certification preferred.

Nice-to-haves

  • Experience in managed care or Healthplan experience strongly preferred.
  • Strong analytical, critical thinking, and problem-solving skills.
  • Ability to present information and obtain buy-in on recommendations.
