About the position
The Patient Services Coordinator-LPN at CenterWell Home Health is responsible for coordinating patient care by scheduling visits and communicating with field staff, patients, and physicians. This role ensures continuity of care and assists with office management, including managing schedules, processing patient visits, and maintaining logs. The position requires strong organizational and communication skills to effectively manage multiple assignments and support the clinical team.
Responsibilities
- Schedule visits and communicate with field staff, patients, and physicians to maintain care coordination.
- Manage patient schedules and edit them for agent unavailability.
- Initiate infection control forms and maintain client hospitalization logs.
- Assist with day-to-day office and staff management, including receiving calls from field staff.
- Prepare weekly agent schedules and run scheduling reports for productivity and geographic issues.
- Perform initial reviews of weekly schedules and forward them to the Branch Director for approval.
- Assist with internal transfers of patients between branch offices and may perform patient visits as needed.
Requirements
- Licensed Professional Nurse or Licensed Vocational Nurse in the state of practice.
- At least 1 year of home health experience.
- Excellent communication skills and ability to interact with diverse individuals.
- Strong organizational skills and ability to manage multiple assignments.
- Valid state driver's license and automobile liability insurance.
Nice-to-haves
- Prior packet review / QI experience preferred.
- Coding certification is preferred.
