About the position
As a Behavioral Health RN Case Manager at Elara Caring, you will play a crucial role in empowering patients to lead healthier lives through personalized care delivered in their homes. Responsibilities
- Manage a caseload of patients, conducting assessments and delivering individualized care.
- Create and adapt care plans tailored to each patient's specific needs.
- Collaborate with physicians, supervisors, and healthcare professionals to ensure seamless communication.
- Educate patients and their families on managing symptoms and care plans.
- Oversee behavioral health aides and LPNs, ensuring care meets the highest standards.
Requirements
- Current, unrestricted RN license in the state.
- Minimum of 1 year of clinical nursing experience in behavioral health, home health, or hospice.
- Reliable vehicle, valid driver's license, and auto insurance.
Nice-to-haves
- Familiarity with Homecare HomeBase software.
