About the position

The Psychiatric Case Manager at Erie County Medical Center is responsible for providing skilled care to inpatient or outpatient behavioral health patients. This role involves working collaboratively with a team of professionals to assess, plan, and implement treatment for individuals facing psychological and chemical dependence issues. The position requires direct supervision by licensed professionals and focuses on delivering therapy services, crisis intervention, and support to patients and their families.

Responsibilities

  • Perform behavioral health assessments and develop treatment plans using established methodologies.
  • Participate as a primary counselor to individuals, in groups, and for families.
  • Participate as a team member in the treatment planning process.
  • Participate in the intake and screening of new patients.
  • Collect and maintain appropriate data from various sources to establish an overall approach to the patient's mental and social problems.
  • Provide a psychosocial history for patients.
  • Provide counseling services to help patients adjust to their disabilities and social environment.
  • Provide individual treatment and/or crisis intervention under clinical supervision.
  • Provide support and guidance to the patient's family to accomplish treatment goals.
  • Observe and record patients' behavioral and psychiatric status, activities, responses to treatment, and overall health condition, reporting to the team.
  • Modify treatment plans as needed.
  • Plan with the treatment team and community agencies to relate the patient's treatment and rehabilitation program to their social situation prior to discharge.
  • Coordinate outpatient services with appropriate community resources.
  • Coordinate discharge planning for patients as assigned.
  • Participate in the development and implementation of daily program activities.
  • Participate in the process of transferring patients to other levels of treatment as appropriate.
  • Refer clients to other governmental and non-governmental agencies.
  • Participate as a member of the team to provide quality assurance and peer-review of the treatment delivered.
  • Assist in the orientation of new staff and in staff development programs and in-service training.

Requirements

  • Possession of a Master's Degree in Social Work, Counseling, or a substantially equivalent field as determined by the New York State Education Department.
  • Possession of a license and current registration as a Licensed Master Social Worker (LMSW) or a Limited Permit to practice as LMSW.
  • Possession of a license and current registration to practice as a Mental Health Counselor or a Limited Permit to practice as a Mental Health Counselor.
  • Possession of Basic Life Support (BLS) Certification from an approved ECMCC provider within sixty days of appointment.

Nice-to-haves

  • Experience in interviewing and assessment techniques for individuals with challenging mental health behaviors.
  • Good knowledge of community resources for individuals with challenging mental health behaviors.
