The Palliative Care Services Psychotherapist/Social Worker plays a vital role as a core member of an interdisciplinary consulting team, providing essential support to patients and families facing serious illnesses. This position encompasses a wide range of social work services aimed at enhancing the psychosocial well-being of patients, including assessments, treatment planning, and therapeutic interventions.
Responsibilities
- Provide psychosocial assessments for patients and families dealing with serious illness.
- Develop and implement treatment plans tailored to individual patient needs.
- Conduct therapeutic interventions and crisis interventions as necessary.
- Facilitate resource referrals and short-term counseling for patients and families.
- Offer grief support and assist with advance care planning.
- Lead family meetings to discuss care options and support needs.
- Collaborate with an interdisciplinary team to ensure comprehensive patient care.
Requirements
- LICSW certification is required.
- Experience in providing social work services in a healthcare setting.
- Strong skills in crisis intervention and therapeutic communication.
