About the position

The Public Health Intervention & Outreach Specialist focuses on community health by providing intervention and outreach services to prevent and mitigate communicable diseases, particularly in the context of HIV and STDs. This role involves engaging with individuals, conducting investigations, managing case services, and promoting health education within the community.

Responsibilities

  • Completes investigations and interventions for communicable disease testing, including blood draws.
  • Conducts interviews and assessments of Public Health District (PHD) patients.
  • Provides ongoing case management services and explains PHD programs to clients.
  • Links patients to social service and health care agencies.
  • Persuades and motivates clients to take necessary health actions.
  • Maintains patient records and databases, evaluating client progress and lab tests.
  • Provides counseling support to clients.
  • Assists with community outreach and health education programs for clients and their families.
  • Documents outreach, intervention, and client service delivery.
  • Manages inventory of supplies and equipment and performs office duties as needed.

Requirements

  • High School Diploma or GED
  • 1 year of experience in social services or public health programs
  • Texas Driver's License
  • Specific technical certifications in public health may be required.
