About The Position

The Public Health Nurse I position is an entry-level role within the Public Health Nurse series at the County of Sonoma's Department of Health Services. This role involves managing a caseload of approximately 25 primary clients and their children, providing individualized assessments, ongoing follow-ups, and targeted health interventions. Requirements

• Possession of a California State Public Health Nursing Certificate.
• Experience working in community, clinic, and public health settings.
• Working knowledge of modern public health practices and community-based resources.
• Ability to work independently and in close collaboration with a multi-disciplinary team.
• Experience working with diverse populations, individuals facing substance use, mental health issues, homelessness, and trauma.

Nice To Haves

• Bilingual English-Spanish is highly desirable, but not required.
• Experience as a Nurse Home Visitor or in Nurse-Family Partnership (NFP).
• Cultural knowledge and experience working with Native and Indigenous communities is highly desirable.

Responsibilities

• Manage a caseload of approximately 25 primary clients and their children.
• Conduct home visits and outreach, typically checking in with two to three families per day.
• Collaborate with families to develop and follow individualized action plans.
• Coordinate and follow up on referrals to other health and social service providers.
• Maintain accurate, detailed, and up-to-date records of all client interactions, assessments, action plans, and referrals.
• Attend mandatory team meetings and periodic training sessions.
• Participate in regular outreach activities.
