About the position

The Public Health Program Coordinator for Family Health at Dakota County plays a crucial role in integrating research and best practices into programs aimed at childbearing individuals, families with young children, and adolescents. This position involves community outreach, program assessment, planning, implementation, and coordination of public health initiatives, while collaborating with various partners to address community health needs.

Responsibilities

  • Inform and guide evidence-based home visiting, surveillance, outreach, and case management.
  • Assess community-wide target population health needs and resources.
  • Initiate assigned policy and program planning, coordination, and implementation, integrating research and best practices.
  • Plan, implement, and evaluate projects, including continuous quality improvement and assurance initiatives.
  • Develop and implement standardized documentation of assessment, interventions, and outcomes in an electronic health record.
  • Work to advance inclusion, diversity, equity, and access in collaboration with colleagues and the community.
  • Collaborate with internal and external partners to facilitate high-quality, holistic service delivery.
  • Evaluate program outcomes, including interpretation and validation of statistical reports for ongoing planning, analysis, and establishment of service priorities.
  • Develop and complete reports, plans, grant applications, forms, and communication tools as delegated.
  • Provide direct public health services for small home visiting caseload.
  • Perform public health emergency response functions as trained and assigned within the department's emergency response plan.
  • Participate in the hiring process and provide a lead role in orienting new public health staff, students, and other division employees to specific program goals and services.
  • Participate in, facilitate, and plan meetings, as well as lead workgroups and strategic planning initiatives for Family Health and departmental programs.
  • Coordinate and facilitate nursing student contracts, clinical placements, and rotations across the department.

Requirements

  • Knowledge of the healthcare delivery system.
  • Knowledge of federal, state, and local laws, rules, regulations, and policies pertaining to public health and professional practice.
  • Knowledge of data privacy and HIPAA regulations.
  • Working skills in program evaluation, data collection, documentation of progress, and outcomes of public health programs.
  • Ability to identify and incorporate best practices and evidence-informed interventions.
  • Ability to communicate effectively both orally and in writing.
  • Ability to exhibit a professional, positive attitude and work ethic.
  • Ability to work effectively with clients from diverse cultural and ethnic backgrounds.
  • Understanding of social, economic, and cultural issues impacting public health programs.
  • Ability to prioritize and coordinate work according to internal and external stakeholder needs.
