About the position

The Public Health Program Specialist II plays a crucial role in assisting families of infants and toddlers with disabilities in accessing essential health and developmental services. This position involves targeted case management, coordination of early intervention services, and collaboration with families and community stakeholders to enhance the early intervention system.

Responsibilities

  • Assist families of infants and toddlers with disabilities in accessing health and developmental services.
  • Make referrals to providers for needed services.
  • Provide targeted case management by coordinating, facilitating, and monitoring the provision of early intervention services under Part C of IDEA.
  • Facilitate and participate in teams to develop, review, and evaluate Individual Family Service Plans (IFSP).
  • Ensure appropriate services are being provided to families.
  • Inform families of their rights and procedures for safeguarding their rights.
  • Maintain early intervention records for accuracy and completeness.
  • Assist families in achieving their goals related to their child's development.
  • Facilitate the development of a transition plan to preschool, school, or other appropriate services.
  • Conduct outreach into the community to identify infants and toddlers with disabilities.

Requirements

  • Bachelor's Degree from an accredited four-year college or university in Early Intervention/Early Childhood Special Education, Early Childhood Education, Child Development, Special Education, or Social Work.
  • 2-4 years of experience in service coordination for children with disabilities or service provision to children under six years of age.
