About the position

The QIDP/Qualified Intellectual Disabilities Professional-Case Manager at Holy Angels Residential Facility is responsible for providing comprehensive case management services to residents with intellectual and developmental disabilities. This role involves advocating for residents, ensuring their needs are met, and maintaining effective communication with various stakeholders to facilitate continuity of care. The position requires a passion for helping individuals with disabilities and includes responsibilities such as overseeing daily needs, coordinating treatment plans, and ensuring compliance with policies and regulations.

Responsibilities

  • Provide all case management services and advocate for residents on the caseload.
  • Maintain close communication with the Habilitation Director and other professionals.
  • Oversee the day-to-day needs of the residents.
  • Report instances of real or suspected abuse or neglect immediately.
  • In-service residents quarterly on their rights and responsibilities.
  • Coordinate with Dorm Supervisors to ensure continuous active treatment as per the Individual Program Plan.
  • Provide in-service training regarding documentation and related matters.
  • Assist managers as requested by the Habilitation Director.
  • Complete all required paperwork within time limits set by the Habilitation Director.
  • Chair meetings for all annual staffing on the caseload.
  • Attend all scheduled meetings as requested by the Habilitation Director or other professional staff.
  • Assist in planning and carrying out social events and departmental activities.
  • Ensure all Habilitation aspects of the survey process are complete and in order.
  • Coordinate with families, staff, and other team members to ensure program consistency.

Requirements

  • QIDP Certification
  • Bachelor's Degree in Human Services/Social Sciences (Psychology, Sociology, Social Work, etc.)
  • 1-3 years of experience working with individuals with intellectual and developmental disabilities
  • Valid driver's license
  • Strong communication skills
