About the position

The QMHP Case Manager at EPIC Health Partners, LLC is responsible for providing mental health skill-building services and case management support to individuals. This role involves ensuring high-quality service delivery, maintaining accurate documentation, and fulfilling care coordination functions. The position requires effective communication and organizational skills, as well as the ability to work both independently and as part of a team.

Responsibilities

  • Complete QMHP duties, including attending supervision.
  • Provide support to individuals in Mental Health Skill Building services.
  • Provide coverage on short notice as needed.
  • Maintain accurate documentation of client interactions, progress notes, and service referrals.
  • Conduct case management and assist with ensuring high-quality services are provided.
  • Fulfill care coordination functions and provide linkage to resources.
  • Communicate effectively with clients and team members.
  • Attend meetings as required.
  • Complete check-ins with individuals.
  • Ensure that progress notes and accompanying documentation are submitted within required timeframes.
  • Ensure service provision within DBHDS, DMAS, MCO, and EHP regulations/requirements.
  • Complete additional administrative functions as requested.
  • Complete other duties as assigned.

Requirements

  • Current registration with VA Board of Counseling as a QMHP-Adult.
  • Must successfully complete a criminal background and central registry check.
  • Valid Driver's License.
  • Current auto insurance.
  • Certified and up to date in CPR and First Aid.
  • Current TB screening.

  • Knowledge of Mental Health Skill Building.
  • Knowledge of community resources.
  • Ability to utilize EHR effectively.
  • Skills to maintain required confidentiality regarding individual case materials in compliance with Human Rights and HIPAA.
