About the position

The ePEP Case Manager-QMHP at Metrocare serves as an intensive case manager and health coach for patients experiencing their first episode of psychosis. This role is integral to the CSC Team, focusing on providing comprehensive support, treatment, and advocacy to help patients achieve their maximum potential. The position involves facilitating psychoeducational groups, coordinating services, and ensuring continuity of care for patients with mental health challenges.

Responsibilities

  • Facilitate psychoeducational and recovery groups using a specific curriculum.
  • Provide overall service coordination, referral, linkage, and advocacy to patients with varying needs.
  • Participate in CSC team meetings.
  • Follow up with assigned patients who missed a scheduled appointment and document attempts to contact them.
  • Follow up within 24 hours on assigned patients who access emergency services and report status to the treatment team.
  • Provide crisis intervention, including home visits and working with police as needed.
  • Assist with inpatient admission and participate in discharge planning.
  • Perform other duties as assigned.

Requirements

  • Master's degree from an accredited college or university in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention.
  • Ability to manage a large caseload of consumers with varying needs.
  • Effective written and verbal communication skills.
  • Ability to work independently with minimal supervision.

Nice-to-haves

  • Bilingual skills, with a stipend of up to 6% of the base salary.
  • Experience in mental health services or case management.
