About the position

The QMHP Community Mental Health Case Manager position at True Mental Health Services is an entry-level role designed for individuals who are passionate about helping those with serious mental illnesses. The role requires a bachelor's degree in fields such as Psychology, Social Work, Sociology, Educational Psychology, or Rehabilitation Counseling, or a license as an LPC-associate or LMSW. The case manager will engage in a variety of tasks aimed at providing community support and case management for clients diagnosed with serious mental illnesses. This position is full-time, requiring 40 hours of work per week, primarily Monday to Friday, with on-call responsibilities as needed. In this role, case managers will have a high volume of direct client contact, traveling to various locations including clients' homes, day programs, doctor appointments, and homeless shelters. The job involves conducting ongoing assessments and planning for clients' mental health needs, educating clients about their symptoms, medications, and coping strategies, and assisting them in accessing necessary services. The training program provided by True Mental Health Services equips employees with the necessary tools to identify client needs and develop effective approaches to assist them. Candidates must be comfortable working independently in the community while also collaborating within a team structure. Strong problem-solving and communication skills are essential, as is a genuine concern for the well-being of individuals facing significant life challenges and mental health issues. The role also includes daily documentation of services provided and participation in team meetings to ensure cohesive support for clients.

Responsibilities

  • Participate in a daily team meeting each morning
  • Travel to 5-6 client locations in the Fort Worth area daily to work with clients in the community
  • Complete 90% of duties independently in the field with clients
  • Conduct ongoing assessments and planning for client mental health needs and education
  • Educate clients about symptoms, medications, and coping strategies
  • Assist clients in completing doctor appointments and discharges from hospitals as needed
  • Help clients access medications from pharmacies and educate them about taking medications as prescribed
  • Provide on-call services (after hours telephone support) on a rotating basis
  • Document daily services provided in client files

Requirements

  • Bachelor's degree or higher in Psychology, Social Work, Sociology, Educational Psychology, or Rehabilitation Counseling
  • Good listening, verbal, and written communication skills
  • Strong work ethic to complete tasks independently
  • Functional knowledge of computer programs (Word, Excel, web-based programs)
  • Good driving record with at least 3 years of driving history
  • Reliable transportation for work (employees will earn mileage reimbursement)
