About the position

Carolina Outreach, an ncgCARE partner, is one of the largest and most respected providers of community-based behavioral services in North Carolina. We are committed to providing a wide array of services to effectively meet the changing needs within the community. Our services include evidence-based practices, school-based and outpatient therapy, psychiatric evaluations and treatment, Community Support, and substance abuse services. We are currently seeking a full-time Qualified Professional or Peer Support Specialist to join our Community Support Team in Charlotte, NC. In this role, the therapist will conduct assessments and provide community-based therapy and case management to adults. This includes conducting initial assessments for clients with serious and persistent mental health issues and connecting them with a higher level of care such as ACT services. The specialist will serve as a member of the client's Community Support Team and will take the lead in the development, coordination, revision, monitoring, and implementation of the client's Person-Centered Plan. Responsibilities also include identifying, planning, scheduling, monitoring, and implementing services for the consumer, as well as coordinating service delivery with other staff and/or across agencies. The specialist will develop transition plans when clients move in and out of services and create therapeutic interventions to work with clients. Written documentation is required, including treatment notes, goals, and plans for the medical record. The specialist must be available to respond as needed to clients and co-workers 24/7/365 and must be free from all illicit substance use. Direct services will involve implementing behavioral interventions, crisis interventions, and rehabilitative and psycho-educational activities. The specialist will work with clients, families, natural supports, and collateral agencies to review, apply, and achieve goals as indicated in the Person-Centered Plan. Community engagement activities related to the client's therapeutic goals and mental health needs will also be coordinated.

Responsibilities

  • Work with clients, families, natural supports, and collateral agencies in reviewing, applying, and achieving goals as indicated in the Person-Centered Plan.
  • Assist and/or coordinate planning and implementing community engagement and other activities related to the client's therapeutic goals and mental health needs.
  • Utilize community-based activities to deliver clinically appropriate services.
  • Provide parent/guardian/natural support training to aid persons interacting with the client.

Requirements

  • High school diploma or GED for Peer Support Specialist; Bachelor's degree in Human Services or related field for Qualified Professional.
  • At least 2 years of experience with the population served for Qualified Professional; 4 years of experience if the degree is in a field other than Human Services.
  • Peer Support Specialist certificate for Peer Support role.
