About the position
The Qualified Behavioral Health Provider (QBHP) at Life Strategies Counseling Inc, part of the Optum family, plays a crucial role in enhancing patient wellness and autonomy. This position involves advocacy, communication, education, and the identification of service resources to facilitate care for patients. The QBHP will be integral to the Optum Behavioral Care Team, contributing to a holistic approach that addresses the physical, mental, and social needs of patients.
Responsibilities
- Coordination of services both within and outside of Life Strategies Counseling (i.e., counseling, medication management, welfare, vocational services)
- Achieve at least 60% Level of Care on individual and collateral visits
- Conduct home visits at least once per month with client and guardian
- Provide transportation as needed for clients
Requirements
- Valid driver's license
- Must be able to travel independently
