About the position
As a Qualified Behavioral Health Provider (QBHP) at Life Strategies Counseling Inc, you will play a crucial role in enhancing patient wellness and autonomy through advocacy, communication, education, and resource facilitation. This position is part of the Optum Behavioral Care Team, which aims to revolutionize behavioral health care delivery and improve the lives of vulnerable patient populations by addressing their physical, mental, and social needs.
Responsibilities
- Coordination of services both within and outside of Life Strategies Counseling (i.e., counseling, medication management, welfare, vocational services)
- Achieve at least 60% Level of Care on individual and collateral visits
- Conduct home visits at least once per month with client and guardian
- Provide transportation as needed for clients
Requirements
- Valid driver's license
- Must be able to travel independently
