About the position

The Qualified Intellectual Disabilities Professional (QIDP) at OURS is responsible for ensuring safe and effective residential services for individuals with developmental disabilities. This role involves overseeing client care, training support personnel, and ensuring compliance with state and federal regulations. The QIDP serves as a positive role model and is expected to work collaboratively with a facility team, requiring rotating weekends and on-call hours.

Responsibilities

  • Ensure that CILAs are in compliance with current policies and procedures, state and federal rules and regulations, and assist with ongoing development, reviews and revisions.
  • Provide feedback and support to direct care staff (DSP's) in conjunction with their supervisor (the House Manager), while maintaining positive working relationships.
  • Attend Person Centered Plan (PCP) meetings organized by the ISC.
  • Develop and implement the implementation strategy plan that coincides with the PCP.
  • Ensure appropriate implementation of outcomes for the residents on your caseload.
  • Follow up with Implementation Strategy Plans (ISP) and ensure that staff is properly executing the outcomes assigned.
  • Review individuals' needs with community support team to ensure appropriate services are provided.
  • Assist or attend House meetings along with House Manager to address concerns or issues.
  • Thoroughly complete quarterly environmental reviews and reports.
  • Complete monthly census for each client of caseload.
  • Complete monthly progress notes and monitor clients to ensure continuity of care.
  • Ensure all files and reports are filed promptly in the client's record.
  • Complete onsite day training visits at least bi-weekly.
  • Monitor clients in their residential setting to ensure appropriate implementation of programs and outcomes.
  • Observe clients for appropriate dress and hygiene prior to leaving for appointments.
  • Review all client records quarterly to ensure timely and accurate documentation.
  • Attend monthly staff meetings, weekly management meetings, and other meetings as directed.
  • Schedule and chair all intake and discharge staffing and special team meetings.
  • Handle complaints from individuals, guardians, agencies, and families.
  • Monitor Therap and ensure Communication Logs are current.
  • Ensure clients are offered opportunities for community integration and that their rights are upheld.
  • Place the safety and welfare of clients first.
  • Complete projects and special assignments as needed.
  • Review clients' status quarterly to determine necessary changes in programming or supports.
  • Ensure all DSPs fulfill the corporate mission.
  • Set a positive example for work ethic and personal demeanor.
  • Report all complaints or grievances made by residents, employees, or visitors.
  • Sign and date all entries in the resident's clinical record.
  • Report all occupational injuries immediately.
  • Assist with hospital hand-offs to ensure rights of individuals are met.
  • Be on-call for incidents, injuries, or behavioral concerns.
  • Provide detailed written summaries regarding medical/behavioral incidents and hospitalizations.
  • Complete annual redeterminations for assigned clients to ensure continuance of state benefits.
  • Communicate individual client status with Finance and Behavior Department for billing purposes.

Requirements

  • Must meet State educational requirements for QIDP (Bachelor's degree in a human service related field).
  • Must become an approved QIDP and DSP trainer with the State of Illinois.
  • QIDP Certification preferred; QIDP experience required.
  • Must have valid driver's license and comply with company policy for driving vehicles.
  • Ability to handle sensitive information and situations discretely and confidentially.
  • Excellent computer skills; Microsoft Office, Excel, PowerPoint, etc.
  • Attention to detail and accuracy of information.
  • Project and goal orientation with strong organizational and interpersonal skills.
  • Excellent written and verbal communication skills.
  • Ability to prioritize and manage multiple assignments in a fast-paced environment.
