About the position

The Mental Health Case Manager plays a crucial role in supporting individuals facing mental health challenges. This position involves conducting assessments, developing care plans, and providing direct support through various therapeutic techniques. The ideal candidate will be dedicated to improving client well-being and collaborating with healthcare professionals to ensure comprehensive care.

Responsibilities

  • Conduct comprehensive patient assessments to determine mental health needs and develop individualized care plans.
  • Provide direct support and counseling to clients, utilizing techniques such as cognitive behavioral therapy and behavioral therapy.
  • Facilitate group therapy sessions, including specialized approaches such as dance therapy and gender issues counseling.
  • Collaborate with healthcare professionals in clinics to ensure holistic care for clients, particularly in geriatrics.
  • Maintain accurate documentation of client progress, treatment plans, and any changes in behavior or condition.
  • Educate clients and their families about mental health issues, treatment options, and resources available within the community.
  • Advocate for clients' needs within the healthcare system, ensuring they receive appropriate services and support.

Requirements

  • A Bachelor's degree in a relevant field is required.
  • A valid QMHP Certification license is required.
  • Experience in mental health counseling or case management is preferred, with at least 1 year of experience.
  • Familiarity with medical terminology and clinical practices is beneficial.
  • Strong communication skills are essential for effective collaboration with clients, families, and other healthcare professionals.

Nice-to-haves

  • Experience working with diverse populations, including geriatric clients.
  • Knowledge of Christian counseling principles may enhance the therapeutic relationship for some clients.
