About the position

The Qualified Mental Health Professional (QMHP) plays a crucial role in the Intensive In-Home team, providing essential interventions in home and community settings for identified youth and their families. This position involves planning, coordinating, and monitoring services to ensure effective support for eligible consumers, while working closely with families to address their needs.

Responsibilities

• Developing and monitoring the implementation of the person-centered plan in various locations such as the recipient's home, school, and community settings.
• Monitoring service delivery regularly to ensure consistency and continuity with the person-centered plan.
• Possessing a working knowledge of child mental health populations.
• Planning and engaging in activities, providing supportive counseling and crisis management.
• Assisting families in meeting educational, vocational, housing, and other basic needs.
• Entering service notes and required documentation into the information management system.
• Attending and participating in meetings on behalf of the consumer.
• Participating in weekly supervisory meetings with the team.
• Participating in clinical training events and other required meetings.

Requirements

• Bachelor's degree in a human service field and 2 years of experience with the mental health population (with at least 1 year of experience with children/adolescents).
• Human service degrees include social work, sociology, psychology, or other human services.
• A non-human service bachelor's degree may be considered if the candidate has 4 years of experience with the mental health population.
• Candidates with a bachelor's degree in a human service field and 1 year of experience with the mental health population can be considered for an Associates Professional position.
