About the position

The Quality Management Program Coordinator will facilitate the development, coordination, implementation, and evaluation of clinical quality improvement activities at BACH. This role involves ensuring compliance with NCQA Patient Centered Medical Home (PCMH) standards and leading initiatives to enhance clinical workflows and patient care quality.

Responsibilities

  • Acquire and maintain expert knowledge of NCQA Patient Centered Medical Home (PCMH) standards, policies, processes, and application requirements.
  • Develop and implement BACH's PCMH model, including organization and submission of materials required by NCQA for recognition as a Patient Centered Medical Home.
  • Coordinate PCMH activities by developing an overall PCMH project work plan under the guidance of the Director of Compliance and Quality.
  • Organize staff and patient feedback groups and build clinic teams for PCMH work plan rollout.
  • Attend and lead internal and external meetings regarding Quality and PCMH matters.
  • Collaborate with providers, staff, and leadership to review and recommend changes to clinical workflows for PCMH compliance.
  • Monitor PCMH compliance and develop new policies and procedures necessary for PCMH implementation.
  • Coordinate training activities related to PCMH data collection and reporting, patient tracking, and workflow changes.
  • Advise management on changes to improve content and delivery of services.
  • Work closely with Electronic Health Records and leadership teams on PCMH requirements and sustainability.
  • Oversee the collection and evaluation of qualitative and quantitative data related to clinical and operational performance.
  • Utilize EPIC and i2iTracks data systems to create reports and review information.
  • Develop strategies to sustain PCMH transformation post-recognition, including sustainability plans and gaps analysis.
  • Research and present information on programmatic tools for PCMH across the agency and community.
  • Work with Clinic Managers to ensure compliance with clinical operations requirements.
  • Meet with providers to review monthly KPIs and overall Clinic Measures.
  • Oversee patient complaints and grievances process.
  • Perform other related duties as assigned.

Requirements

  • Clinical Professional with a degree in Public or Healthcare Administration or equivalent education and experience.
  • Experience in program design, implementation, and evaluation.
  • Knowledge and experience with medical/health quality improvement methods and tools.
  • Excellent verbal and written communication and presentation skills.
  • Ability to multi-task, prioritize, and work under deadlines.

Nice-to-haves

  • LVN, RN, or healthcare degree with knowledge of clinical terminology.
  • Knowledge of and familiarity with NCQA PCMH requirements/application process.
  • Familiarity with EPIC and i2iTracks.
  • Experience working with a Federally Qualified Health Center.
