About the position

The Rapid Rehousing Case Manager at Wayside Youth & Family Support Network is a part-time role focused on providing case management and support to homeless and at-risk young adults aged 18-25. The position emphasizes advocacy, community connection, and assistance in overcoming barriers to self-sufficiency and stability, utilizing various therapeutic frameworks and models.

Responsibilities

• Supporting young adults in accessing basic needs.
• Assessing young adults' stability using Positive Youth Development principles and Trauma Framework ARC Foundations.
• Holding regularly scheduled meetings with young adults in their chosen home communities.
• Networking and building relationships with community partners and resources.
• Managing young adult paperwork including yearly assessments and service plans.
• Participating in area System of Care meetings and collaborating with the Youth Action Board.
• Managing multiple priorities independently in a fast-paced environment.

Requirements

• Bachelor's degree preferred.
• 1 to 2 years of prior experience working with young adults at risk.
• Valid driver's license is required.
• Bilingual/Bicultural candidates are strongly encouraged to apply.
