About the position

The Records Clerk III in Health Information Management (HIM) plays a vital role in ensuring the accuracy, consistency, and completeness of medical records. This position involves coding, auditing, and reporting medical records using advanced EHR technologies, while adhering to hospital and regulatory guidelines. The role requires collaboration with healthcare professionals to maintain high standards in medical record management and compliance with legal requirements.

Responsibilities

  • Analyze charts for accuracy, consistency, and completeness according to hospital and regulatory agency guidelines.
  • Review and process requests for subpoenas from outside providers, agencies, schools, and attorneys in compliance with applicable state laws.
  • Conduct initial analysis on all types of medical reports to verify completeness by reviewing entire medical records.
  • Perform periodic audits to detect and address delinquent outliers in medical records.
  • Assist and advise physicians regarding the records completion process and problem-solving.
  • Complete and process birth and death certificates within required time frames.
  • Verify and abstract demographic and clinical data from patient charts into automated systems.
  • Review requests and subpoenas for medical information, ensuring proper authorizations are obtained.
  • Perform clerical duties in support of medical transcription, including sorting and distributing transcribed reports.

Requirements

  • Proof of Medical Terminology course completion.
  • Two (2) years of hospital Medical Record experience.
  • Typing speed of 45 wpm or as required by the facility.

  • Registered Health Information Technician (RHIT)
