About the position

The Recovery Case Manager (RCM) at Granite Recovery Centers plays a crucial role in supporting clients on their journey to recovery. This position involves regular one-on-one meetings with clients, acting as the primary contact for their families and referral sources, and overseeing the planning and implementation of aftercare. The RCM is responsible for ensuring clients are held accountable for their actions, facilitating communication, and providing necessary support for outside needs such as court hearings and unemployment benefits.

Responsibilities

  • Help hold clients accountable for their actions in the community and the completion of their work
  • Facilitate and provide timely documentation
  • Monitor compliance on client contracts, phone policy, and 'buddy' relations
  • Ensure that all pertinent information is notated on RSS office board at shift change
  • Communicate effectively in written, electronic, and oral form
  • Make sound decisions and/or find alternative solutions
  • Make weekly calls to the client's family and log communication in the EMR
  • Schedule all calls and face-to-face meetings with family members and/or loved ones
  • Conduct family calls within the first 24 hours of admission
  • Make referral calls within the first 24 hours of admission and weekly thereafter
  • Meet with clients at least once a week to monitor the treatment process
  • Execute step downs and discharges
  • Upload/delegate current ROIs and aftercare plans into EMR
  • Attend required meetings including Morning Meeting, RCM Meeting, Supervision, Safety Squad, and Quality Assurance
  • Engage with clients beyond individual case load
  • Introduce new clients on caseload within 48 hours of admission and log interaction in the EMR

Requirements

  • High school diploma or GED required
  • Extensive experience within Mental Health and Substance Use Disorder industry
  • Time management skills and the ability to work on multiple projects simultaneously
  • Ability to coordinate with all departments and oversee clients' stay at GMTC
  • Creation of an after-care plan that realistically meets the needs of the client
  • Vetting of client visitors and phone calls
  • Must meet and maintain all pre-employment requirements including reference checks, background screening, drug screening, DMV check, and license/credential verifications
