About the position

The Recovery Case Manager at Discovery Behavioral Healthcare is responsible for providing intensive case management, care coordination, and advocacy services to promote client stability within the community. This role involves assisting clients with their basic life needs, rehabilitation services, and facilitating their integration into various settings such as school, work, and home.

Responsibilities

• Assistance with meeting basic life needs and improving financial stability.
• Help clients develop and maintain interpersonal relationships.
• Teach skills such as problem-solving, goal-setting, social skills, money management, peer relationships, symptom management, and employment skills.
• Conduct behavioral counseling to assist clients in following plans designed to assist in symptom reduction or resolution.
• Develop, update, and monitor individualized care/treatment plans in collaboration with consumers and their support systems.
• Provide family support and education.
• Assist with group activities or treatment.
• Collaborate with clinical, medical, and administrative teams.
• Engage and communicate compassionately with individuals experiencing mental illness or emotional disturbances.
• Coordinate with other community agencies and systems.

Requirements

• Bachelor's degree in Social Work, Psychology, or a related field is preferred.
• Must hold a current (or in process of getting) Washington State Agency Affiliated Counselor (AAC).
• Valid Washington State Driver's License.
• Computer competency including Microsoft Windows, Word, and other software, with the ability to learn EMR software programs.
