About the position

The Recovery Support Specialist plays a crucial role in assisting members transitioning from intake to their home care team and providing ongoing recovery support services. This position is integral to the treatment team, focusing on the social determinants of health, care coordination, and overall wellness of members during their recovery journey. The role involves working with individuals facing complex social, medical, and mental health challenges, ensuring they receive comprehensive and coordinated care.

Responsibilities

  • Collaborate with a multidisciplinary care team and participate in weekly interdisciplinary team meetings to ensure high-quality, member-centered care.
  • Assist members in identifying goals and support them in achieving their care plan objectives through individual coaching and treatment engagement.
  • Maintain open office hours at the Groups' office location once per week.
  • Provide in-person, virtual, and community support to members as needed.
  • Accompany members to local services such as primary care and mental health appointments, recovery support meetings, and recovery centers.
  • Become knowledgeable about local insurance, healthcare, and social service resources.
  • Link members with resources and follow up to ensure their needs are met.
  • Advocate for members and help them navigate local care and support systems.
  • Participate in group therapy related to peer support and community resources.
  • Maintain timely and accurate documentation in electronic health records.
  • Engage in community outreach activities and education as necessary.

Requirements

  • An associate's degree in a Social Services field is required; a bachelor's degree is preferred.
  • Two years of experience in case management, care navigation, peer support, or similar roles is required; experience with community-based support is preferred.
  • Fluency with office technology, including G-Suite, email, and electronic medical records.
  • Knowledge of motivational interviewing practices and substance use treatment programs.
  • Ability to establish effective working relationships and maintain professional boundaries with staff and community organizations.
  • Excellent written and verbal communication skills.
  • Strong advocacy skills and knowledge of community-based services.
