About the position

The Reentry Program Case Manager at Fresh Lifelines for Youth is responsible for providing structured support and guidance to youth who are on probation or incarcerated. This role involves conducting assessments, setting goals for education and life skills, and preventing recidivism through ongoing case management and community integration efforts. The position requires building relationships with stakeholders and collaborating with community organizations to facilitate activities for clients.

Responsibilities

• Provide ongoing case management for incarcerated youth
• Meet with youth regularly to build rapport and conduct assessments
• Support and coach youth as they prepare for Reentry services and community integration
• Enter data to track youth progress throughout the program
• Plan activities for clients
• Identify and collaborate with representatives from community-based organizations to plan youth activities
• Build and maintain stakeholder relationships
• Prepare agenda for activities and projects
• Provide clients with transportation to and from activities, probation-ordered programs and events as needed
• Provide ongoing case/crisis management for 10-15 Reentry Program Youth
• Conduct intakes with each youth on caseload
• Assist clients and families with locating and receiving outside services to address identified needs
• Engage in community meetings/events to understand the needs of the community
• Attend and actively participate in regular community meetings, MDTs, as needed

Requirements

• Management experience
• Mid-level leadership skills
• Strong communication skills
• Stakeholder management experience
