About the position

As a Referral Coordinator at SonderMind, you will play a crucial role in making therapy accessible to all clients. Your primary responsibilities will include managing referrals from various healthcare systems and partners, ensuring clients receive the necessary support throughout their mental health journey. This position requires effective communication, critical thinking, and a commitment to the SonderMind mission, all while working in a fully remote environment.

Responsibilities

• Interact with external partners, healthcare systems, and clients to ensure access to care for all SonderMind clients.
• Facilitate care coordination activities such as lead and task management, referral processing, and post-session documentation and reporting.
• Assemble and maintain accurate and appropriate documentation for referred clients throughout their mental health journey.
• Manage inbound and outbound calls and other forms of communication while applying standard guidelines to address or route messages to the appropriate care team member for follow-up when necessary.
• Ensure all referrals are complete and processed in a timely and accurate manner via EMRs and other referral sources (cases, faxes, email, phone).
• Utilize a variety of technologies to ensure the client is connected with the appropriate therapist and maintains the required support through an episode of care.
• Conduct outreach to referred clients and partners to obtain critical information to aid in the journey to care.
• Handle other responsibilities and ad-hoc projects based on business needs.

Requirements

• Bachelor's degree or equivalent experience.
• Experience in Care or Referral Coordination.
• Strong desire to assist clients and provide support to those seeking mental health care.
• Strong communication skills, both written and verbal.
• Motivated and eager to learn, with the ability to adapt to new technologies, processes, and workflows.
• Strong problem-solving skills and attention to detail.
• Flexibility to work in a fast-paced, dynamic environment.
