About the position

The position at Mass General Brigham Home Care involves reviewing and processing referral requests for home health services, ensuring compliance with clinical guidelines and regulations. This hybrid role operates primarily during weekdays with some weekend shifts, focusing on delivering high-quality care and customer satisfaction.

Responsibilities

  • Review referral information for compliance with agency guidelines and regulations.
  • Coordinate the referral process to ensure smooth patient transitions to MGB Home Care.
  • Obtain patient assessment information from various sources and transmit it in the appropriate system.
  • Process referral requests in the 4Next system, ensuring completeness and accuracy according to RSC policies.
  • Update and correct clinical and non-clinical data in the 4Next system as appropriate.
  • Develop and maintain collaborative relationships with internal staff to ensure customer satisfaction.
  • Nurture referral source relationships to maintain high customer satisfaction.
  • Support complex admissions using internal clinical and supervisory resources.
  • Communicate pertinent information from MGB Home Care to current medical providers.
  • Achieve account/territory referral and admissions targets.
  • Share educational information and updates with referral sources regarding MGB Home Care benefits.
  • Review referral requests for clinical appropriateness to determine acceptance for services.
  • Reconcile physicians' orders for home care with initial referrals and review for clinical significance as needed.
  • Complete and finalize all referrals with next day SOC by the end of the day.
  • Practice effective communication according to Referral Service Center guidelines.

Requirements

  • Associate degree or equivalent clinical education required.
  • Current MA license as LPN or other clinician (RN, PT, OT, PTA) required.
  • Minimum 2 years of experience in homecare, acute hospital, extended care, case management, or managed care environment required.
  • Computer literate with the ability to learn new software applications.
  • Strong written, verbal, and electronic communication skills.
  • Highly developed interpersonal and communication skills with the ability to interact with a diverse population.
  • Strong organizational skills with an aptitude for obtaining detailed information accurately.
  • Understanding of insurances preferred.
